The hospital bridge
From a hospital bed to a home that fits.

We're the bridge for people who are ready to leave hospital but don't yet have somewhere safe and supported to go. We work directly with hospital teams, discharge planners and SDA providers to line up the right home and the right onsite supports — so leaving hospital doesn't mean starting from scratch.

We partner with trusted SDA providers to find purpose-built housing that matches your funding, access needs and preferences.
TAR3 informed
Complex support, done gently
Our staff are trained in the TAR3 model of psychological first aid — so complex behavioural, mental-health and trauma-related needs are met with skill and calm, not surprise.
Warm handover
Never a cold discharge
We meet participants in hospital, learn what daily life needs to look like, and have workers matched and ready before the discharge date — not after it.
High Intensity Support.
We're expanding into High Intensity Supports — the clinical end of NDIS in-home and SIL care, delivered by staff with the training and supervision to do it safely.
Registered Nurse-led, delegated in line with the NDIS Practice Standards, and paired with our matching system so the workers on shift are the right fit clinically and personally.
